About Vitamin D

Vitamin Dis a group of fat-soluble secosteroids. In humans, vitamin D is unique because it can be ingested as cholecalciferol (vitamin D3) or ergocalciferol (vitamin D2) and because the body can also synthesize it from cholesterol, when sun exposure is adequate (hence its nickname, the "sunshine vitamin").

Although vitamin D is commonly called a vitamin, it is not actually an essential dietary vitamin in the strict sense, as it can be synthesized in adequate amounts from cholesterol by all mammals. More and more researchers consider vitamin D to be one of the most important hormones with multiple functions.

In the liver, vitamin D is converted to calcidiol (25-hydroxycholecalciferol, or 25-hydroxyvitamin D, abbreviated as 25(OH)D) which is the specific vitamin D metabolite measured in serum to determine a person's vitamin D status. Part of the calcidiol is converted by the kidneys to calcitriol, the biologically active form of vitamin D. Calcitriol circulates as a hormone in the blood, regulating the concentration of calcium and phosphate in the bloodstream and promoting the healthy growth and remodeling of bone. Calcidiol is also converted to calcitriol outside of the kidneys for other purposes, such as the proliferation, differentiation and apoptosis of cells; calcitriol also affects neuromuscular function and inflammation.

Vitamin D is available over-the-counter. However, certain medical conditions may increase sensitivity to vitamin D and sometimes hypercalcemia may develop in response to a minimal increase in vitamin D supplementation. At the same time, some patients, even with excessive sun exposure, had a critically low level of vitamin D. As mentioned above, both an excess and a deficiency of vitamin D appear to cause abnormal functioning and premature aging. In order to achieve maximum benefits, the dose needs to be individually fine-tuned.

  • Mortality with Vitamin D - Low blood levels of vitamin D are associated with increased mortality, and giving supplementary vitamin D3 to elderly women has been shown to decrease the risk of death. Vitamin D2, alfacalcidol, and calcitriol not appear to be effective. However, both an excess and a deficiency in vitamin D appear to cause abnormal functioning and premature aging. The relationship between serum calcidiol level and all-cause mortality is U-shaped.

  • Bone Health and Vitamin D - Deficiency of vitamin D causes osteomalacia (called rickets when it occurs in children), which is a softening of the bones. Hypovitaminosis results in impaired bone mineralization and leads to bone-softening diseases characterized by proximal muscle weakness and bone fragility. Beyond that, low serum vitamin D levels have been associated with increased incidence of falls and low bone mineral density. Studies have shown that the supplementation of vitamin D and calcium improves bone mineral density slightly, as well as decreases the risk of falls and fractures in those older than 65 years. The effects of osteomalacia are known to contribute to chronic musculoskeletal pain. Certain Vitamin D preparations were used successfully in management of chronic fatigue and fibromyalgia symptoms.

  • Cardiovascular Health and Vitamin D - Moderate to high doses may reduce risk of cardiovascular disease.

  • Neuropsychological Health and Vitamin D - Low levels of vitamin D are associated with multiple sclerosis. Supplementation of vitamin D may have a protective effect.

  • Cancer and Vitamin D- Low vitamin D levels are associated with some cancers and with worse outcomes in other cancers.

  • Immune Health and Vitamin D- Low levels of vitamin D appear to be a risk factor for influenza, tuberculosis and HIV.

  • Toxicity and Vitamin D - In healthy adults, sustained intake of more than 1250 micrograms/day (50,000 IU) may produce overt toxicity after several months continues intake. However, in our experience some patients needed much higher doses to even to reach low normal level.
